fa طراحی دستورالعمل ایمنی دارویی برای سالمندان بستری در بیمارستان بر مبنای یک مطالعه مروری ده ساله تخصصي Special گزارش مورد case report <div style="text-align: justify;"><span style="line-height:2;"><span style="font-size:14px;"><span style="font-family:IRANsans;"><span style="direction:rtl"><span style="unicode-bidi:embed"><b><span lang="FA">مقدمه و هدف: </span></b><span lang="AR-SA">سالمندان بیش از هر گروه سنی دچار بیماری‌های مختلف و ناتوانی می‌گردند و به همین علت نیاز بیشتری به استفاده از داروهای مختلف دارند. </span><span lang="FA">با توجه به آسیب‌پذیری سالمندان و نقش مهم ایمنی سالمندان بستری در بیمارستان، مطالعه حاضر با هدف طراحی دستورالعمل ایمنی دارویی سالمندان در بیمارستان&shy; عمومی بر اساس مرور ده‌ساله تحقیقات مرتبط انجام‌شده است.</span></span></span><br> <span style="direction:rtl"><span style="unicode-bidi:embed"><b><span lang="FA">مواد و روش‌ها: </span></b><span lang="FA">مطالعه مرور سیستماتیک و مشتمل بر مطالعات جهانی از 2008 تا 2018 در زمینه ایمنی دارویی در سالمندان بود.</span><span lang="AR-SA"> مطالعات طبق معیارهای ورود و خروج وارد و بنا بر معیارهای </span><span dir="LTR">STROB</span><span lang="FA"> (2007) بررسی شدند. در </span><span lang="AR-SA">پایگاه‌ داده‌های انگلیسی شامل </span><span dir="LTR">ScienceDirect</span><span lang="AR-SA">، </span><span dir="LTR">PubMed</span><span lang="AR-SA">، </span><span dir="LTR">Scopus</span><span lang="AR-SA">، </span><span dir="LTR">Web of Science</span><span lang="AR-SA">، </span><span dir="LTR">Medline</span> <span lang="AR-SA">و پایگاه‌ داده‌های فارسی </span><span dir="LTR">SID</span><span lang="AR-SA">، </span><span dir="LTR">Magiran</span><span lang="FA">، </span><span dir="LTR">Iranmedex</span><span lang="FA">، </span><span dir="LTR">Irandoc</span><span lang="AR-SA"> و موتور جستجوی </span><span dir="LTR">Google Scholar</span> با کلیدواژه‌های ایمنی دارویی، سالمند، <span lang="FA">عوامل خطر، بیمارستان، پرستار و کلمات انگلیسی معادل </span><span lang="AR-SA">جستجو شد. داده‌ها در فرم جمع‌آوری داده‌ها ثبت و پس از </span><span lang="FA">جمع‌بندی</span><span lang="AR-SA"> به‌صورت دستورالعمل ایمنی دارویی برای سالمند تنظیم شدند.</span><span lang="FA" style="font-family:"B Mitra""></span></span></span><br> <span style="direction:rtl"><span style="unicode-bidi:embed"><b><span lang="FA">یافته‌ها:</span></b><span lang="FA"> تعداد نمونه سالمندان در مطالعات از 110 نفر تا 385 نفر متغیر بود. از 205 مطالعه موردبررسی، 11 مطالعه دارای معیارهای ورود و خروج به مطالعه بودند که تعداد سه مطالعه در ایران، دو مطالعه در هلند، یک مطالعه در بلژیک، یک مطالعه در برزیل، دو مطالعه در آمریکا، یک مطالعه در پاکستان و یک مطالعه در تایوان انجام ‌شده بود. یافته‌های حاصل از بررسی این مطالعات به‌صورت یک دستورالعمل ایمنی دارویی قابل‌اجرا در بیمارستان با 20 گویه تنظیم شد.</span></span></span><br> <span style="direction:rtl"><span style="unicode-bidi:embed"><b><span lang="FA">&nbsp;نتیجه‌گیری:</span></b><span lang="FA"> مطابق با یافته‌های این مطالعه، یک دستورالعمل ایمنی دارویی برای سالمندان طراحی گردیده است. با توجه به اینکه طراحی دستورالعمل، گام اول اجرایی سازی آن در بیمارستان می‌باشد لازم است طی یک مطالعه پژوهشی و بر اساس آرای صاحب‌نظران مراحل تعیین اعتبار این دستورالعمل بررسی‌شده و هر یک از گویه‌های آن‌ها تأیید و یا اصلاح گردد. سپس به‌منظور حفظ و یا ارتقای ایمنی سالمندان در بیمارستان به کار رود.</span><span lang="AR-SA" style="font-family:"B Mitra""></span></span></span></span></span></span><br> <br> <br> &nbsp;</div> <div style="text-align: justify;"><span style="line-height:2;"><span style="font-family:Times New Roman;"><span style="font-size:14px;"><b><span style="color:black">Introduction and purpose:</span></b><span style="color:black"> The elderly suffer from various diseases and disabilities more than any age group</span><span dir="RTL" lang="FA"><span style="color:black">.</span></span><span style="color:black"> Therefore, they have a greater need to use different medicines. Considering the vulnerability of the elderly and the important role of the safety of the elderly admitted to the hospital, this study was aimed at designing the drug safety instruction for the elderly in the general hospital based on a ten-year review of relevant research and designing the items of a safety drug instruction for the elderly.</span><br> <b><span style="color:black">Materials and methods: </span></b><span style="color:black">A systematic review study was done including universal studies from 2008 to 2018 in the field of drug safety in the elderly. Studies were included according to inclusion and exclusion criteria and reviewed according to STROB criteria (2007). Articles were searched with keywords drug safety, medication, elderly, risk factors, hospital, nurse and equivalent Persian in English databases including ScienceDirect, PubMed, Scopus, Web of Science, Medline and Persian databases SID, Magiran, Iranmedex, Irandoc as well as Google Scholar search engine. The data were recorded in the data collection form. After summarizing, they were adjusted as drug safety instructions for the elderly.</span><br> <b><span style="color:black">Findings:</span></b><span style="color:black"> The number of elderly samples in the studies varied from 110 to 385 people. Out of 11 studies, three were conducted in Iran, two in Holland, one in Belgium, one in Brazil, two in America, one in Pakistan, and one in Taiwan. The findings of these studies were set into a drug safety instruction with 20 items that were applicable in the hospital.</span><br> <b><span style="color:black">Conclusion</span></b><span style="color:black">: According to the findings of this study, a drug safety instruction for the elderly has been designed. Considering that the design of the instruction is the first step in its implementation in the hospital, it is necessary to review the steps for determining the validity of this instruction during a research study, and based on the opinions of experts, and confirm or modify each of their items.
